We’re currently welcoming new patients from Rancho Cucamonga and neighboring communities. With offices in Ontario, Chino, and Rialto, you have three nearby locations to choose from. Ontario Office

Our Ontario office is located at 213 W G St., Suite B, Ontario, CA  91762.

From Rancho Cucamonga, head south toward Archibald Avenue and turn right onto 6th Street (you can take Arrow Route or E 8th Street as alternatives). Follow 6th Street west into Ontario, then turn left onto Euclid Avenue. From Euclid, turn right onto W G Street.

Our office will be just ahead on the left, with street parking available nearby.

Chino Office

Our Chino office is located at 5250 Philadelphia St., Chino, CA  91710.

The drive from Rancho Cucamonga is about 12 miles and usually takes around 20 to 25 minutes. Depending on where you start, you can take I-10 West toward Central Avenue or head toward Archibald Avenue and the 60 West. Once you reach the office, parking is available in the lot directly outside the building.

Rialto Office

Our Rialto office is located at 786 E Foothill Blvd., Suite D, Rialto, CA  92376.

To reach our Rialto office from Rancho Cucamonga, take the 210 East toward Rialto and use Exit 72 for N Pepper Avenue. Continue south on Pepper Avenue toward Foothill Boulevard, where you’ll find our office. The drive is about 18 miles and typically takes around 20 to 25 minutes. Parking is available in the lot outside the office.
